Developmental Disabilities Waiver of ND

Developmental Disabilities provides support and training to individuals and families in order to maximize community and family inclusion, independence, and self-sufficiency; to prevent institutionalization; and to enable institutionalized individuals to return to the community. To achieve this goal, Developmental Disabilities contracts with private, nonprofit and for-profit organizations to provide an array of residential services, day services, and family support services.

The Medicaid Home and Community-Based Services (HCBS) waiver program is authorized in §1915(c) of the Social Security Act. The program permits a state to furnish an array of home and community-based services that assist Medicaid beneficiaries to live in the community and avoid institutionalization. The State has broad discretion to design its waiver program to address the needs of the waivers target population. Waiver services complement and/or supplement the services that are available to participants through the Medicaid State plan and other federal, state and local public programs as well as the supports that families and communities provide.

How do I know if I am eligible for services?

Getting Started

Step 1

Have a developmental disability diagnosis.

If you do not have a diagnosis, please contact Dakota Family Services. https://dakotafamilyservices.org/

Step 2

Apply for the ND Home and Community Based Waiver through Southeast Human Service Center.

Step 3

After you have been found eligible, your DDPM can put in a referral to the organization of your choice to request services.

Step 4

NDAC will review the request and will prompt a meeting date/time to conduct an intake meeting to get services started. 

Step 5

Attend intake meeting. 

Note: Before an intake meeting can be held we will need verification of active Medicaid status. 

Step 6

 Case Manager will develop the plan.

Note: may take 10-15 days to complete.

Step 7

Case manager will send plan to the DDPM for approval.

Note: Approval of the plan is at the state level.

Step 8

Case manager will train selected staff on the plan. 

Step 9

Services begin.

Note: NDAC must wait for state approval.

30 Day Follow Up

This meeting will be held to check in on how services are going and update the plan with information learned during the first 30 days of services. 

Monthly

Case Manager will review and analyze the ISP program data to measure successes and modify the programs as necessary. 

Annually

Each year thereafter an annual team meeting is held to review the plan. Updates and modifications to the plan can be made at this time, as well as the programs goals.
